What is flail chest and pneumothorax?

Flail chest, pneumothorax, tension pneumothorax and chest drains. A flail chest occurs where there are two or more consecutive rib fractures with a resultant instability in the chest wall, it may also occur with a fractured sternum and several ribs.

Is a chest tube and Pleur-Evac needed for flail chest?

A chest tube and pleur-evac will not be needed in every case of flail chest, only in those cases in which the pleural membrane has been torn by a broken rib. Then the CT and pleur-evac would be the route out of any air and blood in the pleural space.

What happens when air enters the pleural space in a pneumothorax?

The pleural space maintains a negative pressure under homeostatic conditions allowing chest expansion to draw air into the lungs. When air enters the pleural space the pressure becomes positive, and the lung can no longer hold its shape and thus collapses. This can be classified as an open or closed pneumothorax.

Why can’t air come out of the pleural space when fractured ribs?

flail chest may or may not involve pneumothorax, that’s why. if the fractured ribs don’t poke a hole in the lung, there’s no errant air to suck out of the pleural space. don’t know how a pleurevac works? this little tutorial started out with a few sample nclex questions someone posted. i answered this one….
